What is the general assessment and intake process?

At LUCREA., our general assessment and intervention process typically involves three stages:

1. Meeting the Family to Discuss the Situation:

  • Initial Interview: We begin with an interview with the family or parents to understand the presenting issues.
  • Preschool/School Clinical Observation: Observations in the preschool or school setting provide additional insights.
  • Liaising with the School: When necessary, we collaborate with the school to gather further information.

2. Assessment:

  • Discussion of Assessments: If needed, we discuss which assessments will help determine cognitive abilities and differentiate issues.
  • Functional Assessment: To assess everyday functioning.
  • Behavioral Assessment: To evaluate the severity of behavioral issues.
  • Attention Deficit-Hyperactivity Disorder (ADHD) Assessment
  • Anxiety Assessment: To allow differentiation between anxiety and other issues.
  • Speech and Language Assessment
  • Comprehensive Report: The assessment process results in a detailed report outlining findings, concerns, and recommendations.
  • Feedback Session: We discuss the report and tailor our recommendations to suit the client’s specific needs.

3. Intervention & Therapy:

Intervention Goals: Using evidence-based therapies, we focus on improving skills such as anxiety and anger management, social skills, and behavioral management training.

Follow-Up Sessions: These are scheduled with the child, parents, and/or family to address the identified issues.

What will happen at the first session?

The first appointment is an opportunity for us to get to know each other. Typically, Program Director or senior therapist will focus on making you or your child feel comfortable and at ease during the session. We will inquire about the reason for your visit, as well as other aspects of your life such as family, school, work, interests, health, friendships, and relationships. This holistic approach helps Program Director understand how your concerns fit into the broader context of your life and how best to support you. By the end of the session, we will provide you with an initial idea of how they can assist and collaborate with you to address any issues.

Are sessions confidential?

At LUCREA Inc., we prioritize the confidentiality of our clients. All staff members, including therapists, interns, volunteers, and external service providers, are required to sign our confidentiality agreement. This agreement ensures that no information obtained during or after their time at LUCREA Inc. is disclosed. All client information is securely stored, ensuring the utmost privacy and care.

However, there are exceptions to confidentiality:

  • When failure to disclose the information would place the client or another person at serious and imminent risk to life, health, or safety.
  • When the client’s (or guardian’s) consent has been obtained.
  • When providing a written report to the client or to another professional or agency (e.g., lawyer, school, doctor).
  • When discussing information with another person (e.g., school, or other professional health services).
  • When disclosure is required or authorized by law.

Although sessions with children and young people are confidential, we strive to involve parents and caregivers as much as possible, with the consent of the young person.

How does the billing process work?

Payment Policy

Thank you for choosing LUCREA, Inc. for your child’s school aide services. Our ability to provide these services relies on the timely payment of accounts. Please read and sign this policy to indicate your understanding and agreement.

Insurance

Full payment is due directly to LUCREA, Inc. We do not currently accept or file insurance. Your insurance policy is a contract between you and your insurance company. Be aware that some, if not all, of our services may be non-covered services and not considered reasonable, customary, and/or medically necessary under your medical insurance plan. You may seek reimbursement for these services directly from your insurance company. If you are unsure about the process for filing a claim, we recommend calling the customer service number on the back of your insurance ID card.

Therapy services provided by our therapists will be considered out-of-network by your insurance company. We will provide you with all information requested by the insurance company, including treatment plans. LUCREA, Inc. is not responsible for any charges your insurance company considers to be in excess of reasonable or customary fees, as well as those considered medically unnecessary.

By signing this policy, you acknowledge your responsibility to pay any amounts not covered or reimbursed by insurance. You accept all financial responsibility for all services provided to the named patient by the therapist and understand that regardless of what the insurance company agrees to pay, you will be responsible for the balance. This balance must be paid regardless of the insurance carrier’s processing status.

Usual and Customary Rates

Our practice is committed to providing the best treatment for our students and we charge what is usual and customary for our area and services. You are responsible for payment regardless of any insurance company’s arbitrary determination of usual and customary rates.

Billing

Sessions are billed monthly. For example, if your child will be seen 2 times per week for 50 minutes each, the monthly (four-week) bill will include 8 sessions of 50 minutes each. Your monthly bill should be paid no later than the 10th of the following month. If your payment is not received by this date, services will cease until payment is made.

Cancellations

All canceled appointments will be rescheduled if possible. Missed appointments affect therapists’ travel and planning time. Please be considerate by canceling appointments within a reasonable amount of time. Our cancellation policy is as follows:

  • Canceled by you with prior notice: Notice must be given to the therapist at least 12 hours before the appointment. If not, the session will be charged at 50% of the normal session rate.
  • No show or last-minute cancellation: The session will be charged at 100% of the normal session rate.
  • Canceled by the therapist: The session will be rescheduled at a time that the therapist is available.
Other Billed Services

We offer additional services, including screening, observation, evaluation, reports, and attending parent-teacher conferences. These services will be billed in addition to the therapy session charges.

Past Due Accounts

Please make timely payments to ensure the continuation of services for your child. If an account becomes past due (i.e., not paid by the 10th of the next month), your child’s therapy services will cease until payment is received.

Method of Payment

You will receive a monthly invoice via email. Please make payments via bank transfer.

  • Bank transfer: Account information is provided on the invoice. (For overseas transfers, a service charge of 2,000 yen will be added to the total amount.)
  • Credit card or PayPal: A 5% additional charge will be added to the bill to cover the fees imposed on us.

Will you see my child alone, or will I be a part of their sessions?”

Decisions regarding session structure are made on a case-by-case basis. For younger children, the first session typically involves the parents unless there is a specific reason for not including them. After the initial session, subsequent appointments may be a mix of individual and joint sessions, depending on the child’s level of concentration and needs during their therapy.


Can I receive a receipt?

Yes, we can certainly provide a receipt. Please provide us with the name and address to which the receipt should be addressed.
